Under 18s Consent Form

I (Dan) am qualified to work with individuals under 18 year olds.

 

I have 20 years experience working with teenagers, have an enhanced DBS check and run a business supporting students with their mental health.

Sessions are run at Pound for Pound Palace, Northam in the presence of other coaches, and security cameras to ensure the safety of everyone involved.

The following form will need to be filled in by a parent / legal guardian before their first session.
